RE: VA COMMUNITY CARE OPTUM PROGRAM – HEARING EVALUATION
In order for you to be seen through the VA CCN OPTUM Program, you must do the following:
* You must go through your PCP/PCM at your local VA. This will ensure you are enrolled in the VA CCN OPTUM Program. “Yearly consults are required to remain enrolled.”
* Make an appointment with your Primary Care Provider at your local VA office. Virtual appointments may be available.
* At your appointment, tell your PCP that you want an authorization through the VA CCN OPTUM Program for a Hearing Aid Evaluation and that you want to be seen at Advanced Hearing and Balance at our location of your choice. This creates the consult.
* Once they submit the consult to the Jackson VA, the Jackson VA approves the authorization, they will fax it to our office, and we will call to schedule your appointment. The VA may also contact you regarding the scheduled appointment. They will mail an appointment letter with your new approved authorization as well.
If you have any questions or concerns, please do not hesitate to contact us at 601-450-0280.
Sincerely,
Advanced Hearing and Balance
